Friendship-West, we did it!!! During the month of October, we did just what we said: provided food boxes and bags for 500 families. And On A Mission kept up the great work distributing meals to over 200 VIPs. The total number of individuals served in October: 1,990. Your generosity and willingness to bless others made this possible. Thank you for assisting us in providing Life Support in such an amazing way.
As we continue to create the beloved community, we recognize that Thanksgiving is right around the corner and so many are still lacking financial resources due to the displacements caused by the COVID-19 virus. With that said, we will be providing Thanksgiving meal boxes to 2000 families. There’s two ways you can help: Donate or volunteer. $2.50 will help us create meal boxes for the 2000 families we plan to serve. Simply go to faithformula.org and scroll down to the Thanksgiving volunteering link or the donate button. We are able to bless so many families because of your partnership and your love for others. So join us as we keep on keeping on showing how amazing Christ continues to be.

FEMA EFSP Emergency Financial Assistance
Faith Formula Human Services is a Section 501(c)(3) nonprofit organization and is located in Dallas, Texas. We currently provide emergency rent or mortgage assistance to the families impacted by COVID-19. Funds are reserved once the following due diligence and safeguard measures are completed: eligibility is established; ALL supporting documentation is submitted and verified; and a funding decision is made. Due to the volume of applications, THERE IS NO NEED TO CALL OR INQUIRE ABOUT THE STATUS OF YOUR APPLICATION. YOU will be contacted as soon as your application is approved.
PLEASE NOTE: By applying for and accepting the funding under the FEMA EFSP (Emergency Food & Shelter Program) through Faith Formula Human Services, you are agreeing that you have not received funding from the following other agencies: Brother Bills Helping Hand; Catholic Charities; City Square; Gateway of Grace; Harmony CDC; Housing Crisis Center; Literacy Achievers; Services of Hope; The Chocolate MINT Foundation; The Turnaround Agenda; Under 1 Roof; or Wilkinson Center. Furthermore, you are also agreeing that if you have received funding from, and/or have been approved to receive funding from, any of the above-referenced organizations, and you neglected to identify this information in the application to receive funding from Faith Formula Human Services for the FEMA EFSP (Emergency Food & Shelter Program), that you may be held liable for the repayment of any and/or all of the funding received for such failure to disclose. |

In times of crisis, we respond by assessing the situation, determining the best route to assist the most people, and galvanizing our volunteers and partners to provide quality service, love and support, and positivity even in tough times.
As we continue to fight this COVID-19 battle, we are doing our best to ensure the safety of our communities and to offer goods, services, and resources to ensure that we not only stand together, but we rise together. Since March 31st, Faith Formula, Friendship-West, and On A Mission have been able to assist 1500 families which equals more than 3300 individuals. On A Mission has served over 800 VIPs. In order to do this, we need your help. Your contributions and prayers can help us continue to feed individuals and families, provide up to date information, and stand with our community as we work to support and shine God's light in the midst of dark times. $1 helps to provide three meals for a family of four. $10 provides a month’s supply for a family of four. Thank you to each person who has given to and volunteered for this movement. It is because of you that we are able to serve others. We are blessed to be a blessing because of your generosity and support. And a special thanks to Sankofa Kitchen and Paul Quinn College for assisting with fresh produce. As we move forward in our new normal, we are finding new and innovative ways to provide care and hope to our communities. If so, The Dallas County Health & Human Services has a Comprehensive Energy Assistance Program (CEAP) for YOU and It’s coming to Friendship-West / Faith Formula Human Services. We are located at 616 W. Kiest Blvd. Dallas, Texas 75224. Please call 214-819-1848 to schedule your appointment.
This is for Dallas County Residents Only! To pre-register please call the Dallas County Health and Human Services Department.
The following items will be needed for utility assistance.
1.Proof of Citizenship (birth certificate, voter’s registration, passport or proof of permanent legal residency)
2. Proof of all income from the last 30 days of said date for all household members (check stub, award letter SS/SSI, etc.)
3. Social Security numbers of all household members
4. ID or driver’s license for the head of household
5. The electric bill(s)
