FEMA EFSP Emergency Financial Assistance
ONE TIME ASSISTANCE PROGRAM
Faith Formula Human Services offers emergency financial assistance to those who have not been affected by COVID-19. Funds are reserved once the following due diligence and safeguard measures are completed, eligibility is established, ALL supporting documentation is submitted and verified, and a funding decision is made. IF YOU HAVE ALREADY RECIEVED ASSISTANCE THROUGH THIS GRANT PROGRAM, YOU ARE NOT ELIGILBE TO RECEIVE A SECOND TIME. THIS PROGRAM ASSISTS CLIENTS ONE (1) TIME. Due to the volume of applications, THERE IS NO NEED TO CALL OR INQUIRE ABOUT THE STATUS OF YOUR APPLICATION. You will be contacted as soon as your application is approved.
In order to qualify you must meet the following criteria:
Applicants MUST be able to provide the following required documents:
- Have not received funds from: Brother Bills Helping Hand, Catholic Charities, City Square, Gateway of Grace, Harmony CDC, Housing Crisis Center, Literacy Achievers, Services of Hope, The Chocolate MINT Foundation, The Turnaround Agenda, Under 1 Roof, or Wilkinson Center.
- Cannot be currently on a voucher program (section 8), on HUD, or living in a relative’s home.
- Must not be more than the current month behind on your rent or mortgage.
- Please note that current month equals the 1st - 25th. Anything after the 25th will be for the following month.

Applicants MUST be able to provide the following required documents:
- LEASE AGREEMENT AND CURRENT LEDGER - Signed by all parties (outline of contract and signatures pages only; first and signature page required).
- FINANCIAL HARDSHIP DUE TO COVID - Examples: Furloughed Letter, 3 months check stubs showing a decrease of hours or pay, 3 bank statement showing loss of income, personal Statement of Hardship or letter from employer demonstrating job loss
- CURRENT INCOME - Examples: Check Stubs, unemployment benefits letter, pay history from an employer, or current bank statements.
- PROOF OF HOUSEHOLD SIZE - Examples: Lease, Tax Return with all in household listed, SNAP letter with all listed, socials for everyone, picture ID cards/passports, school IDs/letters/report cards or signed letter listing all others in a household member
- IDENTIFICATION DOCUMENTS FOR ALL ADULTS IN THE HOUSEHOLD - ID/Driver's license, Social Security Card, Passport, Tax Return with all household members listed, consular card/matriculation card green card, etc.

ROC Program ESG Cares Act Rental Assistance
The Redbird Outreach Collaboration (ROC) program offers direct rental and housing assistance, housing relocation, and stabilizing services to the communities surrounding the Red Bird area of Dallas County. and stabilizing services to the communities surrounding the Red Bird area of Dallas County. This collaboration will assist households facing eviction due to direct and indirect COVID 19 impact by providing emergency assistance to prevent homelessness.

Rent funding is restricted by the following requirements:

- Be at or below 50% of Area Median Income (AMI)
- Reside in the City of Dallas
- Be impacted by the COVID-19 pandemic
- Be housed and “at risk of homelessness" (i.e., at risk of losing housing)
- Not have sufficient resources or support networks immediately available to prevent them from becoming homeless

- Pay stubs for at least the last 30 days to determine gross annual income based on household size
- Proof of income for all household members for the past 30 days (SNAP, TANF, Child Support, and SS Award Letters no older than 30 days old)

- COVID-19 – Certification of Impact of COVID-19 and Need for Assistance
- Housed & At Risk of Homelessness
- Declaration/Assessment of At-Risk of Homelessness
- Risk Factor 3 (Eviction Notice)
- Risk Factor & ( Other Factors)-cost burden, elderly, frail, or living on a fixed income, unemployment resulting in loss of income, lack of assets for emergencies
