Faith Formula Human Services strives to support our community by providing assistance programs as well as referrals to resources within the areas of need. We do not offer all forms of assistance due to our funding types. Completing a submission does not guarantee assistance. Please note, multiple submissions will delay our processes.

Rent and Utility Assistance
Emergency Financial Assistance for rent and utilities for the month of June has now closed.
If you have received assistance through this program within the past 18 months, you will not be eligible to apply.

Those applying for assistance will first answer 10 preliminary questions to determine eligibility. If approved to move forward, an email will be sent to the applicant with instruction on how to proceed and at that time all supporting documentation (listed below) will be required.
Once approved, documents are to be UPLOADED into the ZoomGrants system (emailed or faxed documents will not be accepted).

UTILITY ELIGIBILITY:
- To be considered for utility assistance, you must ONLY owe for the current month (eg. in the month of June, you cannot owe for any other months except for June). All balances prior to the current month must be paid in full prior to applying for assistance.
- Live in Dallas (zip codes beginning with 752 only)
- Meet eligibility guidelines
- Has not received any utility assistance from our organization within the last 18 months
- Utility provider MUST be one of the following:
*Ambit Energy *Atmos Energy *City of Dallas Water *Desoto Water Utilities *Direct Energy *Green Mountain *Just Energy *Reliant Energy *Stream Energy *TXU Energy
RENTAL/MORTGAGE ELIGIBILITY:
- To be considered for rental/mortgage assistance, you must ONLY owe for the current month (eg. in the month of June, you cannot owe for any other months except for June). All balances prior to the current month must be paid in full prior to applying for assistance.
- Live in Dallas (zip codes beginning with 752 only)
- Meet eligibility guidelines
- Rental/Mortgage assistance request must not be for extreme delinquent amounts.
- Has not received any rental assistance from our organization within the last 18 months.
Clients must submit the following:
1. ID or Driver’s License
2. Social Security Cards for all household members
3. Proof of Income (for the last 30 days – includes wages, SSI, SNAP, retirement, etc)
4. Proof of Hardship (explanation of why you are needing assistance)
5. Utility bill (if applying for utility assistance)
6. Lease - (if applying for rent assistance) all pages (not a screenshot from a mobile device)
7. Ledger (if applying for rent assistance)
8. Mortgage statement (if applying for mortgage assistance)
